Position Summary
This position is a full time (1 FTE) direct service Referral Specialist position responsible for the provision of advice and assistance in obtaining medical, social, community, legal, financial and other needed services in accordance with the Orlando EMA Standards of Care document. This position is directly supervised by the Ryan White Project Manager.
Responsibilities and Duties
- Maintain a minimum of 135 Ryan White eligible consumers for medical case management services
- Maintaining orderly files, documenting all activities in Provide relating to the eligibility/referral process and provide accurate information.
- Provide referrals for services as appropriate
- Prepare and participate in Fair Hearings as necessary (when a client requests to appeal the determination of eligibility)
- Have knowledge of federal, state and other local programs in order to refer/link clients.
- Participating in group and/or multidisciplinary team case conferences. Responsible for the documenting case conferences in the consumer’s file in accordance with Orlando EMA Standards of Care.
- Conducting outreach and engagement activities to potential consumers and service providers to promote the availability of eligibility and linkages to services that support the consumer’s access to and maintenance of primary medical care. NMCMs will function along with Peer Mentors as appropriate as consumer navigators to determine barriers to care for and connect or reconnect consumers to HIV/AIDS services.
- Enter each client into the Provide system and update the database as needed
- Participating in Referral Specialist Training in accordance with the Orlando EMA Standards of Care.
- Participating in mandatory and self-selected training and staff development activities in accordance with the Orlando EMA Standards of Care.
- Complete 15 hours of continuing education units annually
- Other duties as assigned
Education and/or Experience
- Associate Degree from an Accredited College in the discipline of social science or health and at lease 1 year of experience working with people living with HIV
- Individuals with an associate or bachelor’s degree in disciplines other than health or social sciences shall have at least 1 year’s experience of direct care coordination for target populations.
- Two years of verifiable experience working with PLWH at an established agency can substitute on a year-for-year basis for an associate degree
Requirements and Physical Demands
- Must have a valid Florida Driver’s License.
- Professional attire required.
- Required to sit or stand for long periods of time.
Schedule
- Monday – Friday, 9:00 am – 5:00 pm.
- Ability to travel as needed (in-town and out-of-town trainings and conferences)
